Fertility Managements: Overview Assessment of ovulation, spermatogenesis and the passages give the evidence that, if found to be satisfactory ovum and sperms hopefully meet together. It is over simplification to expect conception in every cycle in every case. The window for fertilization is very short, few hours. The couple may not have availed those moments for fertilization.

Fertilization depends on the quality of ovum and sperm and their interaction dependent mainly on the penetrating power of the proceeding sperm. Fertilization is a complex biological process in which the sperm penetrates the ovum to be its part. The fertilized ovum then develops and grows to be delivered as a baby. That may not happen in every cycle. In spite of sperm ovum contact, fertilization may not occur, there may be biological barrier.

Baby is multidisciplinary. To have one from the union of two cells must involve innumerable events. It is also important to realize that although fertility is important beginning, it is not end to achieve a healthy newborn. Post fertilization events are equally important.

Treating all the childless couples for fertility is unrealistic. Many may have achieved fertilization but face the post fertilization failure. It is difficult to distinguish between the two but the limitation should not lead to mismanagements.

Treatment strategies should be individualized with maximum cost effectiveness, minimum inconvenience and low risk to the couple. There are instances of women getting pregnant after 10 years of marriage without treatment. It is also a common experience that the treatment fails to achieve conception but the same women is pregnant in another cycle without treatment.

A fertility specialist is well aware that he can neither ensure the infertile couple a child of their own by any kind of treatment nor can deny that they can be parents without treatment. He makes thorough assessment of the circumstances of the individual couple and then offers a treatment by which couples in similar circumstances were successful to have a child of their own. .
